After the quest "Hard Answers", Karliah told me to meet her at the Ragged Flagon, but it never came up in my journal or even came up as a quest. I go to meet her at the Ragged Flagon and she isn't there. Tried going back to the Sanctum where we first met and she was there, trapped behind a door.

Have you done the old trick to get Chillrend early as if you also pick up Mercers plans on the table it will prevent this quest from starting. If you have a save before finishing Speaking with Silence you can replace the plans on the table in the cellar then Hard Answers should work once you finish Speaking with Silence quest.
